Please note: This form is for adult students who wish to indicate their interest in taking courses at College of St. Joseph. If you are interested in entering a program of study at CSJ leading to a degree, you will need to submit an application form to the Admissions office with the necessary support materials and be accepted by the College prior to registering for courses. The application forms are located at www.csj.edu on the home page, under “Forms you will need” or you can contact the Admissions Office at (802) 776-5286 to request a packet be mailed to you.
